An observation is a way of using your senses—most often seeing and hearing—to collect data. Observations are an important part of fidelity monitoring and quality assurance. This observation form for TPP grantees is intended to measure the fidelity and quality of program delivery implementation. The person who completes this form (i.e., the observer) should be someone who attends program sessions and is neither a facilitator nor a participant.
