Observation Tool: Group Activity Facilitation

This observation tool is designed to support growth for Teen Pregnancy Prevention (TPP) Program facilitators by guiding observers in identifying a facilitator’s strengths and opportunities for improvement.

Observers can use the form below, along with the Program Observation Form for TPP Program Grantees required by the Office of Population Affairs (OPA), when observing any facilitator or activity, but may find it particularly valuable for observing new facilitators.
