The goal of preconception (or prepregnancy) care is to optimize health outcomes by providing education and addressing modifiable risk factors. To help clients be prepared for a healthy pregnancy, the American College of Obstetricians and Gynecologists recommends that providers assess for the health conditions outlined in this checklist.
