The Protocol for Responding to and Assessing Patient Assets, Risks, and Experiences (PRAPARE) is a standardized patient social risk assessment protocol from the National Association of Community Health Centers (NACHC). This assessment tool consists of a set of items that can be used to assess and address social drivers (also called social determinants) of health. This web page includes the PRAPARE tool in 27 languages, as well as the PRAPARE Implementation and Action Toolkit to support implementation.
