This log is designed to help support Title X projects provide clients with a place to record daily blood pressure readings and management. This full-page log is accompanied by a wallet-sized version that measures 2.5" x 3.5" when folded, so clients can take it anywhere. The wallet-sized version should be professionally printed to maintain specifications.

Title X grantees and subrecipient agencies are required to have a review and approval process, by an Advisory Committee, for all informational and educational (I&E) materials developed or made available under the project, prior to their distribution. The RHNTC I&E Materials Review Toolkit has resources to ensure compliance with this Title X expectation.
