This checklist is intended to help Title X staff identify vendors to be notified during a clinic closeout. It prompts staff to review contracts for required notice periods and buyout provisions and to ensure notifications are issued in accordance  with the terms of each agreement.

This tool is part of the Closeout Toolkit for Title X Clinics, which supports Title X grantees, subrecipients, and service sites navigate closing out in a way that is timely, compassionate, and mindful of compliance concerns.
