This protocol template includes guidance on assessing and counseling female and male clients as part of Title X Program expectations for providing basic infertility services. Family planning agencies can tailor this template; several decision points, marked as "NOTE," must be addressed before the protocol is ready for use. Family planning agencies should only include information that reflects their own current practices.
