Use this template to keep track of free and low-cost health, social services, and other resources available in the community. This list may include service providers that your agency has formal or informal referral agreements with. 

This tool is part of “Establishing and Providing Effective Referrals for Clients: A Toolkit for Family Planning Providers” which supports Title X grantees’ ability to provide effective referrals to meet the needs of their clients and the Hypertension Prevention and Control Improvement Toolkit.
