The Healthy Relationship Evaluation Toolkit provides resources that grantees and evaluators can use to support evaluation of healthy relationship curricula or interventions that have a substantial healthy relationship component for youth. This toolkit includes a guide and three customizable instruments: 1) a youth survey (as well as a filterable crosswalk of the survey items to specific topic areas); 2) a facilitator fidelity log; and 3) a youth program engagement exit ticket.
