Title X projects must provide for coordination and use of referrals and linkages with primary health care providers, other providers of health care services, local health and welfare departments, hospitals, voluntary agencies, and health services projects supported by other federal programs, which are in close physical proximity to the Title X site when feasible, in order to promote access to services and provide a seamless continuum of care. This sample policy template can be modified with agency- or site-specific information.

This tool is part of Establishing and Providing Effective Referrals for Clients: A Toolkit for Family Planning Providers which supports Title X grantees’ ability to provide effective referrals to meet the needs of their clients.
