Revenue and Expense Worksheet (Monthly Version)

The Revenue and Expense Worksheet helps grantees collect information needed to complete the Family Planning Annual Report (FPAR) and may help with completing the Federal Financial Report (FFR), which must be submitted to OPA on a quarterly basis. There is a a monthly and a quarterly version of the Revenue and Expense Worksheet. Select the version that works for you and your network. For example, when onboarding a new subrecipient, a Title X grantee may want to use the monthly version to ensure that timely training and technical assistance is provided if course-correction is needed. Alternatively, a Title X grantee may want to use the quarterly version if there is stability in their service network and they have established processes to monitor and track revenue and expenses.
