Use this job aid to identify correct coding modifiers when multiple contraceptive-related services have been provided during a single visit. Two sizes of the job aid are available: a page-sized wall chart and a pocket-sized version.

This tool is part of Same-Visit Contraception: An Implementation Guide for Family Planning Providers, which offers strategies and tools that support provision of the full range of contraceptive methods on the same day the client first requests it.
