This tool provides samples of sliding fee discount schedules (SFDSs) that reflect HRSA and OPA Program requirements for health centers that may be Title X grantees, sub-recipients, or service sites. These samples are based on SFDSs used by health centers that receive Title X funding, either as a grantee or a sub-recipient.
