Overview of Mini-Grant Opportunity

The RHNTC will award up to ten mini-grants of $25,000 each to Title X-funded agencies (grantees, subrecipients, or sites) to support implementation of one High Impact Practice Set (HIPS) to address leading causes of maternal mortality and racial disparities in maternal health. Agencies can use mini-grant funds through September 2025 to implement their selected HIPS. The RHNTC’s High Impact Practice Sets (HIPS) for Outpatient Settings Series includes HIPS and accompanying Measurement Tools to:

  • Strengthen hypertension services
  • Address cardiac conditions
  • Care for people with substance use disorder (SUD)
  • Respond to mental health conditions; OR
  • Enhance postpartum services

Each award will be contingent on a commitment to a) implement and document all high impact practices within the selected HIPS, and b) collect and report monthly data using the associated HIPS Measurement Tool. Awardees will be asked to submit: 

  • A baseline assessment of each high impact practice within the selected HIPS
  • Monthly data using the Measurement Tool 
  • An endpoint assessment of each high impact practice within the selected HIPS
  • An end-of-project report documenting progress and achievement status of each high impact practice within the selected HIPS
  • An end-of-project survey to share experiences and lessons learned

Awardees will be asked to participate in:

  • Three support calls with the RHNTC (December, February, and September); and
  • Five group discussion sessions (January, March, May, July, and September)  

Application Narrative and Review Criteria

The RHNTC will use standard criteria to assess applicants’ responses to the following questions: 

  • Initiative: Which HIPS has your agency selected to implement? Who will lead implementation, and how? 
  • Motivation: Why does your agency want to implement the selected HIPS? What impact do you expect it to have? If possible, provide information and/or data that demonstrates the need to strengthen services in this area. 
  • Data: How will you collect each element of data in the Measurement Tool associated with the selected HIPS? Who will be responsible for collecting data and submitting it each month? How will this data be used to strengthen services?
  • Budget: How will the funds be used to support this initiative?

RHNTC Technical Assistance and Support

The RHNTC will announce awards by November 27, 2024. Each awardee will be matched with an RHNTC team member who can answer questions and provide support. As mentioned above, awardees will be invited to three calls with the RHNTC team member to discuss implementation, data collection, award management, and support needs. Awardees will also be asked to participate in a one-hour group discussion session every other month beginning in January 2025. These discussions are an opportunity for awardees to support each other through informal conversations focused on implementation strategies, challenges and solutions, and lessons learned. The RHNTC will schedule additional calls and offer individual technical assistance as needed.
