This quality improvement plan template is designed to help sexual and reproductive health agencies create a strategic QI plan to increase contraceptive access. The plan includes a contraceptive care performance measure tracker and a run chart to observe trends in monthly data.

This tool is part of the Increasing Access to Contraception Toolkit, which supports Title X grantees, subrecipients, and service sites with strategies and resources to ensure contraceptive access.
