Disclaimer: On October 4, 2021, the U.S. Department of Health and Human Services (HHS) Office of Population Affairs (OPA) amended the Title X Family Planning regulations to restore access to equitable, affordable, client-centered, quality family planning services for more Americans. The effect of the 2021 Final Rule is to revoke the 2019 regulations and readopt the 2000 regulations with several revisions to increase access to equitable, affordable, client-centered, quality family planning services. The Final Rule went into effect on November 8th.

This Clinical Chart Review Tool identifies quality indicators that demonstrate compliance with the Title X Program requirements, OPA policy, and QFP. The tool provides a mechanism for service delivery quality assurance and improvement. The clinical chart review tool can be modified to meet the Title X agency’s needs.
